have opted out of insurance. By doing this, we can spend more time focusing on the needs of our patients and
eliminate much of the waste in medicine. Insurance can be used for labs & tests, depending on the insurance plan.
SERVICES
OFFICE VISITS: Initial Adult $150.00. Initial Child $100.00. Following establishing care visit pricing is $50/20 min.
OFFICE VISIT (OV) 20 minutes: simple UTI, Sore throat, follow-up of treatment.
OFFICE VISIT(OV) 40 minutes: Initial well child visit, any visit more than 1 issue to discuss.
OFFICE VISIT (OV) 60 minutes: Pre surgical visit with lab and EKG, New Adult; Mental health; Dietary counselling.
HOURS of Operation: MTR 8-5 in the o>ice. Telehealth, phone calls and classes – W 12-4 and F 9-12
AFTER HOURS: add $50 to the usual charge.

Lab draw: $5.00 + insurance or cost of the lab through Evexia Diagnostics or Lab Corp.
Migraine Abortive treatment/Toradol 60 mg/Promethazine 25 mg: OV + $25.00
Nebulizer Treatment: (OV) + $ 10.00
Pap Smears: (OV) +$45.00; IUD Removal – (OV) $10.00; Nexplanon Removal – OV + $25.00
Paronychia or Ingrown toenail: (OV) +$30.00
Rapid Influenza A/B: (OV) + $10.00
Rapid Strep: Time +$10.00
SIMPLE S SUTURES: fresh wound, not head, neck or groin; (OV) + $ 30.00/laceration
Skin tag removal: (OV) + 1-7 $10.00
Tick removal: (OV) + $ 10.00
Urine pregnancy test: (OV) + $ 3.00
Wart Shave and treatment: (OV) +1-7 $15.00
